Your consent agreements
Agreements with Marli Mynhardt, Specialist Wellness Counsellor. Individual sessions cost R580; couples sessions cost R680. Each session lasts one hour, online via Teams.
Submit the appropriate form after verifying your account. Bookings require at least 24 hours’ notice and payment before confirmation.
Individual counselling
Indemnification
I indemnify the Counsellor from any claims arising from any loss or damage to myself, my family, or my property during and after service delivery.
Payment Responsibility
I acknowledge that the Counsellor does not handle medical aid claims and that I am responsible for the full payment of services rendered.
Payment Terms
I understand that the Counsellor operates on a cash basis, and fees must be paid 24 hours before each session.
Cancellation Policy
I agree that if I do not cancel a session at least 24 hours in advance, I will be charged the full session rate.
Late Arrival Policy
I understand that the Counsellor will wait for 15 minutes after the scheduled session start time. If I do not arrive within this time frame, the session will be considered missed, and I will be charged for the full hour. The session will then need to be rescheduled.
Fees and Quotations
I confirm that I have read the prescribed tariffs or received a written quote and agree to pay the stated fees.
Proof of Payment
I agree that no session will commence unless payment has been made and proof of payment has been received by the Counsellor.
Confidentiality
I understand that confidentiality will be maintained, except in the following circumstances: - If the Counsellor believes I am a danger to myself or others. - If there is suspected abuse of a child, elderly person, or individuals with disabilities. - If disclosure is required by law for legal proceedings.
Consent for Minors
If I am under 18, I understand that written consent from both parents or guardians is required before any counselling can take place.
Substance Use Policy
I agree that if I am under the influence of alcohol or drugs during a session, the Counsellor has the right to terminate the session and reschedule it for another time.
Pre-session Alcohol or Drug Use
I acknowledge that counselling sessions will not be conducted if I am under the influence of alcohol or drugs, and I will refrain from consuming alcohol before or during our sessions.
Understanding of Therapeutic Process
I have read and understand the nature and extent of the therapeutic process.
Communication
I acknowledge that the Counsellor will only respond to emails or messages between 08:00 and 17:00 daily.
Third-Party Information
I understand that no information about the therapeutic process will be disclosed to any third party without my consent.
